We are looking for a new or experienced Band 7 Diabetes Specialist Nurse to join our dynamic community diabetes service serving the London Borough of Enfield. The successful candidate should have the ability to undertake advanced clinical assessments of patients with a diagnosis of diabetes providing appropriate clinical intervention, support, non-medical prescribing ( desirable) , and advice to patients and carers. This role involves clinical decision-making, advanced clinical practice, support with service projects and delivery of scheduled and unscheduled clinical care. The successful candidate will be capable of acting as an expert resource on diabetes and have close liaison with GPs, other members of the integrated community teams, and with social care partners is an essential facet of the role. The service has close liaison with the local secondary care providers, primary care, voluntary organisations and local authorities with the aim of providing a seamless journey for people living with diabetes.

Main duties of the job
Post holders must be able to actively demonstrate and proactively to deliver effective and personal care which is safe for patients in a calm, caring and compassionate manner to every patient, every time.
To perform a comprehensive Diabetes assessment of adults referred to the Diabetes Nursing service with diverse presentations and complex physical and medical conditions.
Use analytical skills to undertake the nurse led consultation and assess the person with diabetes in the community setting and be able to plan, implement and evaluate care in a holistic and inclusive way, ensuring where possible engagement andcooperation with care goals. Care provided will reflect current local and national guidelines.
To provide relevant diabetes educational sessions to other Health care professionals, junior diabetes nurses colleagues, patient and their families, residential and nursing home where appropriate.
To undertake evidence-based audit and a project to further own and teams' clinical practice. Make recommendations to clinical lead or manager where appropriate.
Participate in research and clinical audit within the field of diabetes, both locally and nationally and to disseminate any research findings.
Supervise and support junior members of the team in managing their caseloads.
Use a range of verbal and non-verbal communication tools to communicate effectively with patients and carers to progress treatment programmes

EXPERIENCE AND KNOWLEDGE

- Minimum of 3years post qualification. Experience of providing patient education in an individual or group setting. Knowledgeable of medication used in diabetes.
- Experience in managing a group of patients living with diabetes and completing care plan. Demonstrate relevant knowledge gained in previous employment or elsewhere Knowledge of service improvements.
