Main duties of the job

To carry out planned care for a group of patients without direct supervision.

The post holder will work with senior nurses, matron, consultants and multi-disciplinary team members in monitoring and maintaining excellent standards of nursing care and staff development, as part of the unit team.

Responsibility for the assessment of critically ill patients care needs developments, implementation and evaluation of programs of care.

To work within both the nursing and multi-disciplinary team in the unit to provide evidence based clinical / nursing care to critically ill patients from admission to discharge from the unit.

Monitor patient progress, work towards safe and timely discharge plans to ensure barrier to discharge ae identified and acted on accordingly

To participate in the teaching and supervision of junior staff and student nurses as required.

As part of the managerial and clinical responsibility, take charge of the unit and ensure appropriate treatment and care is delivered to the patients at all times.

Act as a mentor to Band 5's and junior staff below in order to develop their roles both practically and managerially.

To assess the needs of patients and their cares promptly, ensuring that they receive the highest quality of care and work collaboratively with other disciplines to facilitate best practice.

To participate in clinical research and Audits, promote nursing research and recognize how this informs clinical practice.

Ensure units bed state is regularly updated on the hospital Care flow/Acubase and discharge summaries are fully completed before patients are discharged to the ward

Ensure ICNARC and units data collections are promptly entered and completed.

Work collaboratively with nursing senior nurses, colleagues in order to share good practice and provide equity of patient care.

The maintenance of an acceptable environment for patients, reporting and taking appropriate action on hazards and faulty equipment.

Working with the infection control team and facilitate the good hand hygiene, environmental cleaning and participate in all their audit program

Implement the principles of infection control and the prevention of cross infection in relation to patients invasive lines.

Communicate effectively and answer queries to patients, their relatives/carers and the multidisciplinary team members and role model to others this skill.

Care and control of medication & controlled drugs used for patient care, ensuring and monitoring systems used for safe administration, the ordering and maintenance of stock.
