Job summary
The Manager/CNS for anticoagulation services will be required to provide a VTE/anticoagulation monitoring service for patients who are undergoing anticoagulation therapy within the Trust. This will involve the Manager/CNS supporting the Lead Nurse with the implementation of VTE/anticoagulation monitoring across the various clinical business units. The role will involve support to the VTE nurse in undertaking root cause analysis, ensuring effective communication of the process to the multidisciplinary staff, embedding VTE/anticoagulation standard in the care delivered to the patients and monitoring of the effectiveness of service delivered.
The Manager/CNS will predominantly lead and support the anticoagulation clinic nurses for anticoagulation services within the outpatient clinics. Ensure the Trust maintains the 95% CQUIN target set by NHS London. Be responsible for education and training of staff and patients in all aspects of anticoagulation care within the trust.
Support with the collection of VTE data required for the CQUIN target, to ensure information is collected in a timely manner.
The post holder will will manage the Anticoagulant clinic three days a week in the OPD setting to enhance the anticoagulation service.
Main duties of the job
To effectively run nurse led outpatient's clinics alongside the Lead nurse.
To initiate and follow up patients with Warfarin and DOACs and discharge safely to GP care.
Work independently in the absence of a Lead nurse within clinical competency.
To undertake teaching of patient's (carers if appropriate) in the treatment and management of the anticoagulation therapy medicines.
Responsible for patient's safety and high standard of specialist care for all patients on anticoagulation therapy within the service.
To liaise closely with specialist nurses and Lead nurse to ensure the effective triage of referrals to enable patients to access the appropriate element of the anticoagulation service.
Responsible for contributing to specialty clinical governance group.
To implement strategies to facilitate succession planning and retention of motivated staff.
To set up clinics, maintain equipment and be responsible for clients and staff as laid down by Health and Safety regulations.
Act as a role model for junior members of staff.
Involving in helping in initiating DOACs and prevention of VTE (HAT).
Work as part of the team.
To be competent in the anticoagulation therapy software Helicon maintain accurate records of all patient encounters in line trust record keeping policy NMC.
Undertake research projects in line with research skills and knowledge with view to presentation and publication
