Job summary
The post holder will act autonomously to promote best practice in the management of respiratory conditions, such as asthma and COPD, in adults & older people across Enfield. The post holder will work with the rest of the respiratory team in providing training and development to clinical teams within primary care and support community and practice nurses who are involved in the direct management of patients with respiratory conditions.
The post holder will independently manage clinics for assessment and review of patients with complex respiratory conditions and oxygen assessments and reviews as required. The post holder is required to have or to be willing to undertake nurse prescribing.
The post holder will be willing to learn to perform and analyse capillary blood gas and spirometry. The post holder will work closely with colleagues in the community to ensure that the COPD pathway is implemented and maintained so that patients have a clear and managed care plan. The post holder must be able to work sensitively with patients from a range of backgrounds, races and cultures to ensure quality of service is available to all.
Main duties of the job
- To work closely with patients to provide individual care plans that will enable them to self-care wherever possible. To facilitate the use of patient held records where this is appropriate.
- To provide complex case management/oxygen reviews as per BTS guidelines in clinics or within the patient's home.
- To assess use of inhalers with specific patients and ensure that they are using the right technique

- Perform and analyse spirometry results.

- To use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
- To undertake all aspects of clinical duties as an autonomous practitioner, to hold responsibility for own case load and to be responsible for defined areas of the service seeking support from more senior staff if required.
- To work closely with patients to provide individual care plans that will enable them to self-care wherever possible. To facilitate the use of patient held records where this is appropriate.

- To plan, implement and evaluate teaching/training sessions for the multi-disciplinary team including social care providers, therapists, GP teams and community nurses that enable them to understand respiratory disease and to recognise and manage symptoms appropriately.
- To provide formal and informal training, supervision and assist in the performance management for more junior staff. Contribute to the training, support and informal supervision of your peers.
- To be responsible for the supervision, assessment and education of students, technical instructors and support workers.
- To assist physiotherapist in the education delivery of Pulmonary rehabilitation
- To work with patient and carers to provide information advice and support that will enable them to manage their conditions better themselves and provide rapid response to specific patients who require more support.
- To actively promote Flu vaccination amongst at risk clients and their carers.
- To use patient contact as a means of providing health promotion and education especially on smoking cessation, diet and physical activity.
- Ensure that Smoking Cessation advice is given to all patients and staff as appropriate.
