Job summary

We are seeking to appoint a dynamic and enthusiastic candidate to the role of Patient Pathway Coordinator within our Paediatric Clinics Management team responsible for our Child Death Overview Panels (CDOP), Paediatric Inbox, Referral processing for the full department, and processing clinical advise & guidance requests received from primary care providers.

This post is offered as a fixed term position working 37.5 hours per week undertaking a 13 month Level 2 Apprenticeship programme to cover the Maternity Leave period of a member of the team. Note that although a substantive role would be offered following successful completion, this may not be within the Paediatric service.

Main duties of the job

The postholder will join a newly restructured team maintaining clinics management within the service working alongside our Senior Medical staffing team consisting of 20x Consultants, and 3x GP's, as well as Junior Medical staffing clinics, and our Clinical Nurse Specialist team.

The Patient Pathway Coordinator (PPC) will work within a dedicated specialty clinical area to facilitate the smooth running of the patient journey by ensuring that efficient and streamlined administrative processes take place revolving around each patient and their individual needs. PPC's will sometimes be expected to provide cross-cover for other members of the team to ensure a consistent and effective administration service is maintained at all times.

The PPC will assist the patients throughout their hospital experience to ensure that care provided is timely, efficient, and appropriate in regards to all members of the healthcare/ multidisciplinary team, and giving the patient a single point of contact for their appointments, ensuring the patient and carer experience is central to every stage of the patient journey, as well as ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager.
