We are looking for someone with/ or willing to develop Band 7 Diabetes Specialist Nursing skills and experience in diabetes to join our team.
Diabetes care is provided to patients in the form of structured education, in community-based clinics and in their own homes throughout the London Borough of Enfield.
You will be a key member of the Community Diabetes team, responsible for the delivery of expert diabetes care. The team has close liaison with the local secondary care providers and primary care with the aim of providing a seamless journey for the diabetes patients.
Main duties of the job
You should have the ability to undertake advanced clinical assessments of patients with a diagnosis of diabetes providing appropriate clinical intervention, support, non-medical prescribing, and advice to patients and carers.
This role involves significant responsibility for clinical decision-making, advanced clinical practice, support with service projects and delivery of scheduled and unscheduled clinical care.
You will be capable of acting as an expert resource on diabetes and have close liaison with GPs, other members of the integrated community teams, and with social care partners is an essential facet of the role.

- To perform a comprehensive Diabetes assessment of adults referred to the Diabetes Nursing service with diverse presentations and complex physical and medical conditions, to provide a clinical diagnosis and develop and deliver an individualized treatment programme.
- To undertake a comprehensive holistic assessment of patients including those with diverse or complex presentations/multi pathologies. To use advanced clinical reasoning skills and appropriate assessment techniques to provide an accurate clinical diagnosis of their current diabetes issues.
- To hold responsibility for own caseload and be responsible for working without direct supervision. Supervision takes the form of regular clinical reasoning sessions including peer and case reviews. Access to advice and support from a more senior diabetes nurse is available when required.
- To undertake all aspects of clinical and leadership duties as an autonomous practitioner.
- To undertake evidence-based audit and a project to further own and teams clinical practice. Make recommendations to clinical lead or manager where appropriate. Contribute to the implementation of specific changes to practice or contribute to service protocols with support and guidance from more senior staff.
- Promote people with diabetes Equality, Diversity and rights.
- Manage nurse led clinics, virtual online appointments and also managing a telephone clinic/helpline to provide support to people with diabetes, carers and other healthcare professionals. This will require high level of autonomy.
- Use analytical skills to undertake the nurse led consultation and assess the person with diabetes in the community setting and be able to plan, implement and evaluate care in a holistic and inclusive way, ensuring where possible engagement and cooperation with care goals. Care provided will reflect current local and national guidelines
- Involvement of the wider MDT must be sought as clinically indicated.
