Job summary

This is an exciting opportunity to join the Royal Free London Group Tobacco Dependency Team. We are recruiting Tobacco Dependency Advisors who enjoy working as part of small teams delivering the inpatient tobacco dependency pathway on the following sites: Barnet Hospital, Chase Farm Hospital, North Middlesex University Hospital and the Royal Free Hospital. The postholder will be expected when required to travel and work within their remit across all sites within the Royal Free Group.

Main duties of the job

The Tobacco Dependency Advisors will:

Support the implementation of the Ottawa Model of care in an inpatient acute setting, delivering 1:1 Tobacco Dependency interventions. Complete follow up phone calls for clients at 7- and 28-days post discharge from hospital Liaise with Community Smoking Cessation services to manage referrals. Support the Population Health Programme manager in integrating the tobacco dependency pathway as part of routine care. Use professional expertise to develop a personalised suitable support plan for everyone, referring to community services as necessary. Organise and deliver training to ward staff, reinforcing the training provided to in-patient teams by the RFL Faculty of Population Health to embed prevention across the Trust. Work with patients, staff and visitors to encourage behaviour change, providing advice, resources and participating in health promotion events across the Trust. To competently use trust IT systems to update patient records and service outcomes. Support the Population Health manager to organise and attend health promotion events, in line with National Campaigns.
