Job summary
This specialist nursing role encompasses the core dimensions of expert clinical practice, education, leadership, management and research within the Rheumatology service. The post holder will work as part of the multidisciplinary team to deliver high-quality, patient-centred care to individuals with rheumatological conditions under the guidance of the Lead Nurse and wider clinical team.
The role involves supporting the assessment, monitoring and ongoing management of patients throughout their disease pathway, including assisting with the initiation and optimisation of treatments where appropriate. The post holder will provide education, information and ongoing support to patients and carers to improve understanding of their condition and promote effective self-management.
Care will be delivered through face-to-face consultations, telemedicine clinics and structured drug monitoring programmes to ensure safe and effective treatment management. Opportunities will also exist to contribute to national outreach initiatives and collaborative work with other trusts.
The role supports the development of specialist knowledge across rheumatological and autoimmune diseases, including systemic sclerosis and connective tissue diseases, whilst developing advanced communication, assessment and clinical decision-making skills to manage a defined patient caseload with increasing autonomy within the multidisciplinary team.
Main duties of the job
Clinical Responsibilities
Act as a key point of contact for patients and carers from referral onwards, providing ongoing support throughout the patient pathway.Provide specialist advice and education on rheumatological conditions, including disease processes, prognosis, treatment options and lifestyle optimisation to support self-management.Undertake nurse-led clinics within agreed competencies, supporting the assessment, monitoring and management of patients with rheumatological conditions.Perform wound assessment and deliver appropriate wound care and dressings in both outpatient and inpatient settings in line with best practice and Trust policies.Provide specialist rheumatology nursing care and support ward-based nursing and medical teams in the management of patients with rheumatological conditions.Work collaboratively with the MDT team to ensure effective communication and coordinated, patient-centred care.Provide clinical advice to GPs, practice nurses and other healthcare professionals internally and externally via telephone advice services, written communication or F2F discussion.Apply clinical judgement to review investigation results and contribute to the planning, implementation and evaluation of care for patients with complex needs.Undertake nursing procedures in accordance with Trust policies, clinical guidelines and professional standards.Maintain accurate, timely and comprehensive patient records using electronic and written documentation systems.
