Job summary

The post-holder will work within the Pharmacy Clinical Commissioning Team to ensure all financial and commissioning data for high cost medicines are reported to the Trust Finance and Commissioning Departments to ensure timely billing to external commissioners. This will include ensuring the Trust database is maintained with respect to external reporting requirements including but not limited to patient details, medication (including form, dose, and amount of medication supplied), indication and any other requirements requested by the Trust or the commissioners. In addition, the post holder will assist in the provision of additional Pharmacy databases which are used to inform external commissioners.

In addition, the post holder will be required to support the Pharmacy Homecare Service Senior Assistant Technical Officers if requested by the Pharmacy Business and Finance Manager

All Senior Assistant Technical Officers must have achieved NVQ level 2 or be studying for the appropriate accreditation.

Main duties of the job

  1. To ensure that all pharmacy commissioning records are complete and accurate and are attributed to the correct medical team or Consultant.
  2. To ensure that Clinicians are aware of and follow the agreed Hospital Formulary.
  3. To ensure that all patient demographic details (address, postcode, GP address, etc) are checked on a quarterly basis and to make any amendments once changes have been confirmed.
  4. Provision of a high quality reporting to commissioners which allows patients to continue their medications without risk of withdrawal of any agents due to information gaps
  5. To develop, implement and maintain Standard Operating Procedures (SOPs) for own area and to contribute to policies when appropriate.T
  6. o work with managers and colleagues to continually improve the quality of services within the overall clinical commissioning governance framework.
  7. To ensure that all patients receiving externally commissioned medications have in date approvals from the relevant commissioner.

1. CLINICAL RESPONSIBILITIES

The delivery of high quality pharmacy clinical commissioning information.

To ensure that all pharmacy commissioning records are complete and accurate and are attributed to the correct medical team or Consultant.

To ensure that Clinicians are aware of and follow the agreed Hospital Formulary.

2. RESPONSIBILITY FOR PATIENTS

To ensure the safe, effective, and economical use of drugs for all patients within the Trust

To ensure that all patient demographic details (address, postcode, GP address, etc) are checked on a quarterly basis and to make any amendments once changes have been confirmed.

To manage difficult situations involving members of the public.

Provision of a high quality reporting to commissioners which allows patients to continue their medications without risk of withdrawal of any agents due to information gaps;

To analyse and interpret prescriptions and data collection forms and either resolve problems with the clinician responsible for the patient or refer problems to the specialist pharmacist.

To know the limit of their knowledge and when to refer information requests on to more senior colleagues

To be the liaison between The Royal Free London NHS foundation Trust and external commissioners where additional information requirements are identified.

3. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT

To monitor the service on a regular basis and to identify and manage any changes necessary to maintain a high-quality responsive pharmacy clinical commissioning service.

To develop, implement and maintain Standard Operating Procedures (SOPs) for own area and to contribute to policies when appropriate.

To follow all departmental SOPs.

To liaise with the multi-disciplinary teams, clinical nurse specialists, consultants and pharmacy staff as required

To work with managers and colleagues to continually improve the quality of services within the overall clinical commissioning governance framework.

To support in the preparation and circulation of paperwork for relevant Multidisciplinary meetings

4.RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES

To ensure that all patients receiving externally commissioned medications have in date approvals from the relevant commissioner.

To be responsible for matching written information with electronic and faxed information for all externally commissioned medications.

To be responsible for identifying discrepancies between agreed prices e.g. contract prices, and the price invoiced by the company.

To be responsible for photocopying and filing all clinical commissioning paperwork.

To maintain a record of supply to these patients. To prepare computer-generated reports, match with the drug notes and file in the individual patient folder.

To maintain the databases for patients receiving treatment for all externally commissioned medications, adding new patients and updating existing patient data, as necessary, according to information received from the specialist nurses or pharmacists etc. To remove data for patients who have stopped treatment and archive it.

To look up the details of new patients addresses and telephone numbers etc. on the Hospital Patient Administration System.

To set up patient computer and hard copy files for these patients and others, supplied by specialist pharmacists.

To support the control of medicines expenditure via:

i. Monitoring drug use and expenditure

ii. Supporting the implementation of the local formulary

iii. Managing drug stock levels and avoiding drug wastage

iv. Participating in any therapeutic switching alternatives

v. Ensuring correct documentation of high-cost drugs on pharmacy high cost drug database.

vi. Participating in any pharmacy/Trust wide cost improvement activities

5. RESPONSIBILITY FOR LEADING AND MANAGING

To work closely with senior clinical commissioning colleagues to manage and support the development of Pharmacists, trainee pharmacists Doctors, Consultants, Nurses, Technicians, Student technicians and Senior Assistant Technical Officers.

To produce and frequently update commissioning procedures.

To identify and prioritise therapy services to individuals and groups of pharmacist according to agreed policies and service.

To implement and maintain an effective system for tracking the progress of commissioning requests.

To act as an effective role model and to provide pharmacy clinical commissioning advice and support to members of the multidisciplinary team

To use a degree of initiative and deal with matters independently where possible but to refer non-routine enquires to a more senior member of staff.

6. RESPONSIBILITY FOR INFORMATION RESOURCES

To be familiar with the pharmacy homecare databases;

To accurately input data into the pharmacy databases;

To maintain the administration of all patients receiving highs cost medicines this includes capturing the registration, suspension, re-instatement and discontinuation of patients treatments where appropriate

To act as a super user for the Automated Dispensing Machine inputting new barcodes and updating the database;

To help with the departmental requirements of filing, scanning, archiving, or shredding paperwork, as necessary.

To cover the specific clinical commissioning duties of the other Senior Assistant Technical Officers or to help them if their workload is excessive at any one time.

To keep the price list updated for medications which are externally funded and the delivery and services costs for specific treatments.

To help staff in all sections of the department, in the lead up to the annual stock take, to ensure all stock levels are correct and discrepancies investigated.

To help compile the monthly report for Finance and Commissioning departments for all high cost drugs supplied to patients. To email copies of the report to designated members of the hospital staff by 5th day of each month and supply hard copies, as required

To compile reports as directed by The Pharmacy Business and Finance Manager in a timely manner.

To be responsible for setting up the physical files for new patients, to file the new patient profiles and remove and archive the files for patients who have stopped a specific treatment. To archive so that information can easily be found, if needed in the future. To reorganise the electronic and hard copy filing information containing the details of the above patients, if/when expansion of the number of patients being treated for a specific disease, makes this necessary.

To ensure patient confidentiality is maintained at all times when dealing with agencies outside the Trust.

If required to cover for the Homecare and MTA senior technical assistants, to ensure the invoice is correct, to enter the data on to the pharmacy JAC computer for payment purposes and book the drugs to the patients consultant, for stock control and budget purposes.

If required to cover for the Homecare and MTA senior technical assistants, to check the invoices from the transport companies against the records made and pass the invoices for payment. To investigate and verify any record, on the invoice, which pertains to transport ordered by any other section of the pharmacy.

7. RESPONSIBILITY FOR RESEARCH AND DEVELOPMENT

To lead on the identification, evaluation of and promotion of suitable and relevant training opportunities for technical staff in clinical commissioning

To assist in the identification of training needs for the development of technical staff

Maintain personal development CPD records for staff development record
