Clinical Team Leader - Hospital@Home

Job summary

We are seeking a dynamic and enthusiastic candidate to join our Paediatric Hospital@Home service for a 12-month fixed term contract for Maternity Leave cover as Clinical Team Leader.

This role is offered at 15 hours per week as part of a job share and the successful candidate will work 2x shifts per week as part of a roster agreed with their line manager.

The H@H service is commissioned by NCL ICB and has provided positive patient experience and outcomes to children in their own homes, who otherwise would have been admitted to Inpatient units. The function of the team will be to provide hospital standard care to children with acute nursing needs in the community.

Working within a virtual ward model, the service will develop bespoke care pathways for children requiring nursing care which will avoid extended lengths of stay and admission. The team will work closely with Paediatricians and pharmacy colleagues to ensure a seamless transfer home and provide high quality nursing care according to their treatment plan.

Main duties of the job

The BCYP H@H operates 7 days per week, 08:00 to 20:00. The service currently offers 7 care pathways, and it continues to develop new pathways. The Hospital at Home service sees babies, children and young people at home for treatment and assessments.

The post holder will be supported to manage the H@H Service, implement pathways and lead on the day-to-day running's of the service. They will work within the multidisciplinary team to provide safe and appropriate management of children and young people with an acute illness which can be treated in the community. They will be working closely with the acute teams to ensure that patient safety.

The post holder will have vast experience in paediatric nursing, and be able to to identify patients suitable for H@H , to support patient flow and rapid discharges from ED or the wards. The post holder should be able to plan, implement and evaluate patient care, as well as have some management experience, or be willing to learn and develop management skills.
