Job summary
- The role is within a multidisciplinary team to support patients exercise safely and effectively following Myocardial Infarction, Coronary Artery Bypass Surgery, Valve repair/replacement and Percutaneous Coronary Interventions in both secondary care and potentially community venues.
- To support the Cardiac Rehabilitation team in the supervision and exercise activity of patients attending Phase 3 Cardiac Rehab programme.
- With support from the lead physiotherapist explore the development of the physical activity and exercise program for phase 4 Cardiac Rehabilitation for Enfield service users in accordance with its protocol in community setting or secondary care.
- The post will be for a fixed term of 12 months to assist in the development, support, and delivery of the established program.
- To provide a high standard of clinical exercise activity working as an integral part of the cardiac rehabilitation team delivering a menu-based programme to the priority group of patients suitable for cardiac rehabilitation.
- You will deliver the programme in a variety of settings with specific emphasis on the delivery of exercise testing, prescription, and delivery.
- You will participate and support in audit/data collection for the purpose of service evaluation and improvement.
Main duties of the job
- Lead and co-ordinate the supervised exercise sessions of the CR programme & liaise with CR colleagues to ensure effective use of facilities for the structured exercise sessions.
- Conduct comprehensive physical activity assessments pre and post the programme, for all patients & partners recruited to the programme.
- Devise, implement, monitor, and progress patients' & partners' individualised exercise prescription and a progressive home-based exercise programme, based on thorough assessment utilising policy, documentation, and knowledge of specific medical conditions.
- Monitor patients & partners to provide group or 1 to 1 support by additional appointments, when necessary
- Plan, deliver and review patient and family/carer education sessions.
- Facilitate behaviour change for all components of the preventive cardiology programme in all patients & partners by active participation in health promotion.
- Liaise closely with CR colleagues to provide exit route opportunities and encourage long term sustainability of an active lifestyle through participation in Phase IV or mainstream provision as appropriate.
- To be aware of the breadth of physical activity opportunities available across the borough and the required level of fitness appropriate to each
- To have planned approach to receiving feedback from patients/clients to continuously improve the service provided.
- Negotiate with patients around goal setting and behaviour change.

The Cardiology team provides a comprehensive adult inpatient and outpatient cardiology service, delivered by three clinical/professional groups: Medical, Nursing, and Cardiac Physiologists. These groups are supported by a team of Care Pathway Coordinators, a Service Manager, a Deputy Service Manager, and additional support staff.The Nursing service encompasses a Catheter Laboratory and Cardiac Day Case Unit, Nurse-led Pre-assessment Clinics, Elective DC Cardioversion, Tilt Table Testing, a Nurse-led Rapid Access Chest Pain Clinic, a Cardiac Chest Pain Hot Clinic, Nurse-led Post-PCI Clinics, a Nurse-led Heart Failure Service for both inpatients and outpatients, and Cardiac Rehabilitation.
