Job summary
This is an exciting opportunity to join the Multidisciplinary Team at Finchley Memorial Hospital Community Diagnostic Centre (CDC) to develop diagnostic-focused non-specific symptoms and Upper GI pathways.
The post holder will be responsible for the smooth running of the patient journey from initiating referral, diagnostic procedures, MDT meetings, diagnosis and onward referral by ensuring that coordinated and streamlined clinician and administrative processes revolve around the patient and their individual needs.
This post is subject to completion of clinical training programme prescribed by Consultant GP Diagnostician, this must be successfully completed to progress to a band 6 post.
Main duties of the job
Assess, plan, implement and evaluate individualised patient care throughappropriate frameworks.Assist members of the team in the co-ordination of the clinical activities. Responsible for the MDT management of patients on specific pathways includingpreparation, coordination, documentation, and follow up of administrative actionsfrom the MDT meetings, to ensure all patients are discussed in line with the NHS.Cancer Plan. This includes administrative improvements to facilitate themanagement plan for patients discussed at the MDT
