Frailty Practitioner for Geriatric Emergency Medicine Service (GEMS)

Job summary

This is a full time Band 7 post working in a newly established frailty service at the North Middlesex University Hospital NHS Trust. This role is part of a key development in delivering specialised frailty input to frail, older patients presenting to ED.

The post holder will be included in the 7-day working. The successful candidate will work across Urgent and Emergency Care within the GEMS team and contribute to the delivery of timely, specialist input to frail patients; effecting appropriate admission avoidance and early, pro-active CGA for patients who may need admission to appropriate wards.

Main duties of the job

  • To assess, develop, implement, and evaluate specialist care programmes to ensure frailty syndromes are fully optimised
  • To obtain people's informed consent, providing highly specialist advice and discussing the implications of the whole assessment process
  • To undertake specialist interventions consistent with evidence-based practice, transferring and applying knowledge and skills to meet patients' needs, evaluating and modifying interventions as appropriate.
  • To analyse and rigorously review all aspects of the patient care plan interpreting information and using specialist knowledge and clinical judgement to liaise with MDT (Multidisciplinary Team), co-ordinating and participating in case discussions as required
  • To refer patient/family/carer to other practitioners when needs and risks are beyond one's own scope of practice or require longer term support
  • To support patients/family/carers encouraging them to promote their own health and wellbeing and to express their interests and concerns
  • To provide support and care for the patient and his/her family/carer respecting their need for privacy and dignity
  • Establish and maintain effective communication with patient/carers and families about complex matters providing a warm welcoming and enabling environment to support therapeutic relationships.
