Job summary
The CNS contributes to the provision of specialist palliative care in the NorthCamden community for patients facing life-limiting illnesses (within the remit ofthe multi- disciplinary team) accepting patient referrals to agreed criteria andmanaging their own caseload with the support of the Lead Nurse andConsultant Nurse. They will provide advice, education and support to patients,their carers and other health and social care professionals. They will contributeto the development and smooth running of the service. The post holder willwork to agreed professional, departmental and Trust boundaries as required toprovide patient centred seamless care and will take part in the 7 day / weekservice to both community and hospital patients.

You should be creative, enthusiastic, and motivated with:
Broad and extensive post registration experience of working with acutely sick patients, emotionally distressed patients and their families, and terminally ill adults.Excellent interpersonal skillsThe ability to work autonomously and as a team memberHold a first degree and post registration qualifications in oncology, palliative care, community nursing or other appropriate medical speciality.Demonstrate extensive teaching experience.
