Job summary
You will work as part of amultidisciplinary team to provide high quality, integrated nursing care to patients within the HIV outpatients and day care service. You will assist in the assessment and monitoring of the patient's disease pathway. Initiating, optimising treatments and will provide information, education, training, and support to this group of patients and their carer's.
Providing a holistic service via face-to-face consultations, telemedicine clinics, facilitating drug monitoring programmes and work collaboratively to provide screening programmes for defined patient groups.
Main duties of the job
This specialist practise role will incorporate the dimensions of expert clinical practice, education, leadership, management, and research.
The post holder will work as part of a nursing team to provide care within the HIV outpatients and day care service and will work closely with other staff across all disciplines to facilitate continuity of care.
The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care in whichever part of the department they are working on both a daily and ongoing basis.
The post-holder is responsible for providing competent treatment, care and advice to promote health and well-being. Their role will include triage, provision of day care and treatments, providing nursing support to health care support workers co-ordinating out-patient clinics and facilitating HIV testing, screening programme's and diagnosis.
The post-holder is designated to take charge of the department on a shift-management basis acting as floor manager, supervising junior staff and liaising with the nurse practitioner and the lead nurse as necessary.
