Job summary

The post holder will be an integral member of the multi-disciplinary team, they will provide support signposting, advocacy, and mentoring to patients to work through personal action plans and help navigate around health and social care services. Additionally, they will review and further develop health, welfare, peer support and social care services for people living with HIV attending the Ian Charleson Day Centre (ICDC). The post holder will liaise and work with acute medical and voluntary sector organisations to establish links for people living with HIV to a range of support services e.g. advice on housing, immigration and financial issues. In addition the post holder will be a point of contact for HIV queries from other areas of the hospital, including providing a liaison role between our inpatient service and Emergency Department (ED) in particular. The Royal Free has a 'support hub' which has provided some support to people attending ICDC in the past and liaising with this service is also expected to provide some continuity in providing holistic support. The Royal Free London NHS Foundation Trust is aiming to become HIV stigma free. We anticipate the post holder playing an active role in helping the Trust achieve this goal.

Main duties of the job

The HIV service at the Royal Free London NHS Foundation Trust is recognised as an International Centre of Excellence for HIV care. It serves the inpatient and outpatient needs of over 3000 people living with HIV. We have an inpatient HIV unit and run specialist consultant-led clinics within the department including viral hepatitis, TB, cardiology, renal disease, rheumatology, ageing and frailty, chronic fatigue, lipid disorders, AIN, gynaecology and psychiatry. For stable patients on medication and e-clinics are in place, which are both Doctor and Nurse led. Care in all areas of medicine is increasingly being provided at a network level and the department is active in developing and strengthening links with our clinical colleagues across the North Central London Network. The HIV department has a dedicated research team, and many patients are involved in Clinical Trials including those for new antiretrovirals, primary HIV infections, and questionnaire-based studies. The servicehas an integrated clinical psychology team and routine wellbeing review clinics have recently been implemented. A weekly peer mentor from Positively UK also supports the service.
