Job summary

The TREAT nursing team operates across a wide patient pathway: the Trust's Emergency Department, the Trust's care of the elderly in-patient wards, and across local Care Homes and community services. The key objectives of the TREAT nursing team are to:

  1. Support care of all medical specialty patients >= 80 year of age, attending the Emergency Department.
  2. Support care of patients attending the Emergency Department based 'Hot Clinics'.
  3. Prevent inappropriate readmissions of elderly patients from Care Homes, post discharge from the elderly in-patient wards at the Royal Free Hospital.
  4. In collaboration with the Community Hub service reduce avoidable re-attendance to the Emergency Department or re-admission to in-patient services via the Community Hub service.

On a rotational basis the TREAT Senior Staff nurse role will rotate around the key areas of the TREAT nursing service to provide expert clinical nursing support for elderly patients:

  1. Emergency Department (admission avoidance)
  2. Elderly medicine Short stay units ( Adult Assessment Area and follow-up care post-discharge )

About us

  1. TREAT will identify patients suitable for follow-up at the TREAT Hot clinic; transport to and from attendance at the Hot Clinic will be supported by the TREAT.
  1. Contact patients (or their significant other) post discharge from Emergency Department and at appropriate intervals within their 31 days of discharge from hospital to ensure that discharge plans are fully implemented.
  2. Record findings and ensure TREAT medical teams are kept fully informed of progress post discharge.

Job description

Job responsibilities

  1. In collaboration with the medical staff, identify all medical specialty patients 80 years of age and assess if suitable for referral to the TREAT service.
  2. Coordinate all aspects of social, medical and nursing care needs for all TREAT patients within the Emergency Department..
  3. Identify and escalate to medical staff any patients admitted to the Emergency Department or Hot Clinics who are on the Community Hub current patient list
  4. Advise Emergency Department team, as required, on discharge processes to support elderly non-TREAT patients.
  5. In collaboration with the TREAT Registered Nurse based in the Hot Clinics, supervise at all times, the TREAT Nursing Assistant.
  6. Re-allocate nursing resources to ensure needs of the Emergency Department and Hot Clinics are supported as required.
  7. Follow-up TREAT patients 24-48 hours after attendance in Emergency Department. Communicate progress of discharge patients to TREAT medical staff and escalate to community services as appropriate. Maintain follow-up database.
  8. Identify patients suitable for referral to Community Hub; on identification, refer the patient to the TREAT Community Hub nurse.
  9. Support nursing staff in the Emergency Department Adult Assessment Unit (AAU) to ensure smooth discharge of elderly patients.
  10. Attend TREAT Hot Clinics to provide expert clinical nursing support, e.g. medication administration and wound dressings.
  11. Supervise the Hot Clinic based TREAT Healthcare Assistant to ensure patients receive all aspect of care e.g. monitoring, nutrition and hydration, toileting, etc.
  12. Support referrals to Community HUB Service and presentation of patients at Community HUB Meetings.
