Senior pharmacist - Clinical Commissioning and Sub-Regional Immunoglobulin Assessment Panel (SR-IAP) Band 8A

An exciting opportunity has arisen for a Senior Pharmacist to lead the North Central London Sub Regional Immunoglobulin Assessment Panel (NCL SR-IAP) which includes Trusts from across NCL. We are seeking a highly motivated, experienced Pharmacist with knowledge of Immunoglobulin processes to coordinate, develop and maintain the SRIAP and develop the team and the service provided to internal and external commissioning partners.

Main duties of the job

The post holder will be responsible for managing requests of Immunoglobulin made to the NCL SRIAP, clinically reviewing applications for the region and will be the contact point for NCL-SRIAP associate trusts and external commissioners. The post holder will also be responsible for delivering value for money and working with colleagues within the Trust to promote patient safety and achieve better outcomes and improved value from the use of High Cost Drugs (HCDs) relating to NHSE. They will also play a role in the continuous development of medicines optimisation initiatives at the Trust. It is expected that the successful candidate will have excellent information technology knowledge.

Job responsibilities

1. CLINICAL RESPONSIBILITIES

To provide a high quality clinical pharmacy service as part of the multidisciplinary team to patients, to ensure maximum efficacy, safety and economic drug use and to demonstrate professional accountability to enhance patient care. The responsibilities will include:

  • Regular attendance at consultant ward rounds and multidisciplinary team meetings where applicable
  • Demonstration of expert Immunoglobulin as well as a general clinical knowledge.
  • Delivery of a high quality, specialist ward-based pharmacy service. This involves obtaining medication histories from patients, clarifying inconsistencies with the patients GP or Community Pharmacist, ensuring appropriate use of patients own drugs, drug supply, review of highly complex medication regimens, clarification of medication changes made during inpatient stay by liaising with the medical team, drug interaction and adverse drug reaction monitoring, discharge planning and rapid turnaround of TTAs, full medication counselling to patients, and assessment for appropriateness of compliance aids to assist patients medication needs.
  • To make clinical decisions where limited/conflicting information is available.
  • Promotion of high quality and cost-effective prescribing.
  • Making evidence based decisions, in partnership with patients and other professionals within the specialist area.
  • Liaising with other pharmacists to ensure continuity of medicines management, and to implement systems for the effective transfer of medicines between different care setting.
  • Development and review of Shared Care guidelines
  • Implementation of relevant local, regional and national guidelines within the specialist area particularly with respect to Immunoglobulins.
  • Assist in the implementation and evaluation of Drug Usage Reviews and clinical audits, and implement changes in prescribing practice accordingly.
  • To liaise and work with other specialist pharmacists to share the best practice, to strengthen patient focused pharmaceutical care.
  • To practice as a supplementary/independent prescriber within a defined scope of practice within the specialist area.
  • To assist in the implementation of e-referrals and electronic Medicines Management within the specialist areas.
  • Support the monitoring of the use of High-Cost Drugs, with a specialist interest in Immunoglobulin, ensuring on-going compliance with clinical guidelines and eligibility criteria.

2. RESPONSIBILITY FOR PATIENTS
  • To assist in the planning and organising of a clinical pharmacy service to the patients in both the inpatient and outpatient settings.
  • Develop a detailed understanding of NHS England IFR and IVIG procedures and support appropriate applications to the relevant panels, with a specialist responsibility towards the NCL SR-IAP.
  • To assist in identifying and managing changes necessary to maintain a high quality service responsive to patients needs and to consider the implications of changes for other areas/personnel.
  • To provide clinical screening and dispensing support to the pharmacy team to ensure the accurate and timely dispensing of prescriptions
  • To deal effectively with situations where medication errors have occurred, including liaising directly with patients and staff concerned.
  • Assist in responding to complaints from patients and the public in relation to medicines management issues as required.
  • To deliver a high quality and comprehensive medicines counselling service to patients and/or their relatives on all aspects of their pharmaceutical care including specialist and unlicensed drugs. To overcome communication barriers through the use of interpreters or other forms of communication.

3. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT
  • To support and Coordinate the North London Sub-Regional Immunoglobulin Assessment Panel (SR-IAP)
  • Create the minimum dataset required to enable patients to be discussed at the SR-IAP and ensure all patients for discussion meet these requirements
  • To develop the pathway that ensures patients are discussed at the SR-IAP in a timely fashion.
  • Work in conjunction with the Commissioners to establish KPIs of the Panel, as well as how and when progress against these KPIs will be monitored and reported
  • To provide efficient and timely Secretariat support for the SR-IAP
  • To assist in the production and implementation of business plans for the development of the pharmacy service provided to the SR-IAP.
  • To assist in the formulation, prioritisation and implementation of long term strategic plans for clinical commissioning and immunoglobins use in line with local, network and national objectives.
  • Contribute to the development of and ensuring adherence to evidence based prescribing guidelines including production of evidence reviews for Immunoglobulin request for non-routinely commissioned indications.
  • Ensuring compliance with CQC medicine related outcomes, NICE medicines appraisals and other regulatory related initiatives
  • To ensure that all pharmacy activities, procedures and documentation within the speciality are carried out in accordance with standard operating procedures and relevant medicines legislation

4. RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES
  • To contribute to the management of the NHS England budget designated to the NCL SRIAP with oversight from the Principal Pharmacist-Clinical Commissioning.
  • To contribute to the management and reporting of commissioner funded medicines. The responsibilities will include:
  • Work with NHS England CMU to provide detailed evaluated reports on Immunoglobulin usage and expenditure for the NCL SRIAP associated trusts.
  • Identify options for reducing drug expenditure in line with savings programmes.
  • Forecasting new drug developments and identify changes in prescribing practice that may impact on regional drugs budgets.
  • Maintain local databases for high cost drugs to monitor usage and ensure funding is in place to reduce financial risk
  • Interpreting published data on new Immunoglobulin preparations or alternative therapies for new or existing clinical indications.
  • Liaising with the key decision makers (senior managers, Senior Clinicians, Chief Pharmacists, Senior Pharmacists and Commissioners) to enable best demand management of Immunoglobulin for the NCL SRIAP associate trusts.
  • To provide advice on Immunoglobulin and other high cost drugs to facilitate the seamless transition of patient care between interfaces or care settings by liaising with consultants, Service Performance and Primary Healthcare Professionals.
  • Contribute to, monitor and report on quality / cost improvement programmes for High Cost Drugs, with a specialist interest in Immunoglobulin.
  • Provide management, mentorship, support and assistance to pharmacy staff in connection with the NHSE medicines optimisation strategy and quality / Improving Value Schemes.
