Job summary
A great opportunity has arisen for an experienced Clinical Nurse Specialist to join our Pulmonary Hypertension service at the Royal Free London Hospital. We are recruiting a band 8a lead nurse for a one-year fixed term period. Leading and contributing to the strategic, clinical, and operational managementof the service. This opportunity will suit a dynamic individual with band 7 experience in advanced nursing practice, service development and line management.
Main duties of the job
You will be responsible for Nursing leadership, inspiring and developing the Clinical Nurse Specialist workforce and foster and develop strong collaborative working relationships internally and externally. You will gain experience in service development, research and have the scope to be innovative.
We welcome individuals from a variety of disease areas, such as cardiology, respiratory and Rheumatology, as your clinical and management skills will be transferable. You will be part of a supportive multidisciplinary team and training in the disease area will be provided.

JOB SUMMARY:
- The Pulmonary Hypertension (PH) Service at the Royal Free is part of the national service within the UK. The service is a designated treatment centre for PH by NHS England to diagnose, treat and monitor PH patients. As one of only eight adult centres in the UK, patients with suspected PH are referred to the Trust from across the England and Northern Ireland. Patients are treated with targeted commissioned therapies which may be given orally, intravenously or inhaled
- Develop new and improved care pathways through leadership and innovation within the trust and externally
- Contribute to service strategies for the PH patient pathway, quality standards, evidenced based practise and national shared care partners
- Facilitate the provision of high quality, autonomous, integrated nursing care to PH patients, in line with NHS England service standards
- Represent the nursing workforce in relation to pulmonary hypertension care at appropriate groups/committees both internally and externally
- The post holder is expected to spend 0.6 (WTE) of the role in clinical practise and undertake a clinical caseload
- Demonstrate an advanced level of knowledge, advanced assessment skills and expertise in clinical care, treatment and management of patients with PH, incorporating the physical, psychological and social impact on patient care
- Lead, negotiate, supervise, participate in, and manage a caseload of patients within nurse led clinics and service as a whole
- In conjunction with the multidisciplinary team (MDT) facilitate the pathway of care which includes relevant professionals within the Trust and external stakeholders for the management of patients requiring treatment for PH
- Liaise with the MDT, re MDT meetings, patient pathway tracking and communication to external Trusts
- Initiate/review relevant investigations, imaging and tests in conjunction with the MDT and with agreed guidelines
- Monitor and review the effectiveness of interventions with the patient and colleagues and modify this to meet changing needs and established goals of care
- Facilitate safe access to services should an acute situation arise by developing safe care strategies and effective workforce planning
- Attend ward rounds, to review and facilitate care pathways for PH patients on the ward
- Facilitate nursing care at shared care clinics by attending an agreed number of external clinics, held at other Trusts per year
- Make advanced, evidence-based decisions, in partnership with patients and other professionals, using agreed protocols, but also where there are no appropriate precedents
- Initiate, titrate and monitor PH therapies as per treatment plan and patient group directives
- Discuss/review patients on a weekly basis with the multi-professional team to address patients health needs through planning and delivering interventions which are based on best practice and clinical judgement
- Liaise with Home Care providers and Commercial Medicines Unit (NHSE) to ensure regular supplies of disease targeted therapies, pumps and consumables and contribute to national homecare framework specification
- Assess patients for suitability and supervise six-minute walk tests during out-patient clinics and prior to cardiac catheterisation
- Provide clinical information, advice on the use of specialist equipment and advice to patients, carers and other healthcare professionals through a telephone advice line/email service and escalate appropriate queries to the MDT
- Act as an advocate and empower patients, their families and other carers to participate in decisions concerning their care
- Monitor and audit all patients receive holistic needs assessment at the appropriate intervals in the pathway
- Complete advanced communication skills modules and proactively seek/attend clinical supervision sessions
- Ensure safe and timely follow up of patients back to local hospital / shared care team following diagnostic tests and treatment initiation or discharge
- Document and maintain accurate records in line with professional standards of record keeping.
