CLINICAL RESPONSIBILITIES
Monitor the appropriateness of blood and blood component usage in association with the blood transfusion laboratory via the use of audit methods. Under supervision or following appropriate training provide specialised transfusion advice, which contributes to the care of patients. Provide an effective link between the Transfusion Laboratory and medical/nursing staff so that problems and adverse incidents can be fully investigated. After appropriate training to act as a resource to medical and nursing staff, supporting, and giving advice/information on aspects of transfusion practice. Work with blood transfusion laboratory staff and associated teams to manage operational issues relating to the remote blood issue system.
RESPONSIBILITY FOR PATIENTS
Ensure that patients complaints are handled on accordance with the Trusts Complaints policy after appropriate training. In line with Trust policy and Caldicott / Data Protection Act requirements, maintain patient confidentiality. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT Investigate and develop ways of reducing blood and blood component use in a safe cost-effective manner. To monitor and improve standards of care via audit, evidence-based practice, and research. Work with other members of the transfusion team to help develop transfusion standards that improve quality of the transfusion service offered to patients. Participate in Serious Hazards of Transfusion reporting or analysis as required. Participate in the development of multidisciplinary audit of transfusion practice, perform audits and work with HTC and the HTT to improve the quality and safety of the service provided. Utilise the current evidence and best practice in all areas of clinical work. Liaise with key staff to help develop transfusion standards that improve the quality of the Transfusion service offered to patients. In collaboration with Blood bank ensure that non-compliance with statutory requirements for traceability is followed up, monitored, and escalated appropriately. Contribute to and update policies / guidelines / protocols for blood transfusion With the Lead TP, plan and implement evidence-based protocol and guidelines covering all aspects of the transfusion of blood and blood products and ensure these guidelines are effectively disseminated. In conjunction with the Lead Transfusion Practitioner review and re-issue protocols and guidelines as and when required. Assist in formulating long term strategy plans for blood usage to ensure adequate supplies of blood and blood products in times of national shortage.
RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES Produce and update Transfusion information on the Transfusion laboratory Intranet site. Ensure the related patient information leaflets are available in clinical areas.
RESPONSIBILITY FOR LEADING AND MANAGING
Participate as a key member of the Hospital Transfusion Committee and HTT. Work closely with the Consultant Haematologist (Transfusion Medicine), the Transfusion Department and members of the HTT and the HTC Participate and facilitate meetings related to blood transfusion. Help to ensure that learning from mistakes is fully communicated to all clinical teams and that action plans are implemented and fully complied with. Participate in clinical governance structures in ensuring any concerns are notified and communicated appropriately. Working with the transfusion team to assess the training needs of medical, nursing, and multidisciplinary staff with regards to the safe administration of blood and blood products. Support and undertake training programmes based on the Trusts Blood Transfusion Policy, for all medical, nursing, and multidisciplinary staff involved on the transfusion of blood and blood products and ensure that all staff receives regular updates. Participate in staff induction days to introduce best practice for blood product use. Work in collaboration with the Trust Training and Development Department to develop and implement appropriate educational training programmes. To participate in developing a shared vision of the service and work with the multidisciplinary team, organisation, and external agencies to achieve this. To negotiate agreement with individuals, groups and other practitioners outcomes, roles and responsibilities, and action to be taken to develop resources, services, and facilities as directed by Lead TP. To maintain appropriate channels and styles of communication to meet the needs of patients, relatives and carers, managers, peers, and other professions / agencies. To employ effective decision-making skills to address complex issues and use effective change management skills to implement these. To use effective prioritization, problem solving and delegating skills to manage time effectively. To establish networks with other specialists at local, national, and international level, to exchange and enhance knowledge and expertise. To maintain a peer network of support, information and learning with other transfusion practitioners within the organization. Support Lead Transfusion Practitioner in interpreting professional and regulatory Transfusion related policies from external sources e.g. NHSBT, NHSLA, NMC, DoH and contributes to the development of recommendations and implementation of changes to clinical guidance or transfusion systems as necessary.
RESPONSIBILITY FOR INFORMATION RESOURCES Investigate report, document, and close adverse transfusion incident/near misses, in line with divisional incident investigation processes and the Trusts Clinical Governance strategy. Produce, analyse, and present reports on blood transfusion, as requested Participate in auditing adherence to national, regional, and local transfusion policies and guidelines. Continually develop a regular, updated programme of education in transfusion matters for all relevant staff. To participate in the development and coordination of the multi-professional educational programme, delivering training in transfusion policy and undertaking auditing of the policy.
