Job responsibilities

• Work across Royal Free London Hampstead site as an autonomous specialist, supporting the provision of high quality care for patients in the vascular virtual ward.
• The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the specialty.
• To facilitate the provision of high-quality specialist care to patients, treatment and follow up care in the vascular Virtual ward to outpatient setting.

• Liaise with and refer patients to community nursing teams for Vac therapy and manage the patient journey on the virtual ward, clinic appointments and then discharge to community team once all equipment is in place.
• Provide accessibility and troubleshooting advice for patients once discharged on the vascular virtual ward pathway by carrying a contact phone (in working hours).

• Provide liaison with industry partners (3M) for staff training, consumable levels and educational opportunities.

• Develop this new nursing post based on patient experience feedback, audit and research.
• To facilitate the building of relationships with community tissue viability teams through the upcoming transition period, working towards the standardisation of referral processes across the NCL sector

Education & professional Qualifications

Essential

• First level registration
• Mentorship course
• Evidence of continuing education

Experience

Essential

• 3 years post registration experience of acute nursing
• 18 months experience caring for Vascular patients
• Experience of complex discharges
• Experience of teaching / mentorship/ preceptorship
• Experience of working with students in the last year

Skills and aptitudes

Essential

• Advanced communication skills
• Ability to prioritise and lead
• Interested in continuing to develop practical skills
• Understanding of basic managerial skills
• Able to identify clinical strengths and weakness
• Understanding of preceptorship, mentorship & staff development
