Job summary

The post holder manage a group of patients from in patient setting to outpatient setting via the virtual ward. The post holder will work closely with the surgical team to find suitable patients to be admitted to virtual, then complete the referral for community vac therapy, apply prevena vac, admit patients to the virtual ward and work closely with the community Nursing teams

Main duties of the job

  • Work across Royal Free London Hampstead site as an autonomous specialist, supporting the provision of high quality care for patients in the vascular virtual ward.
  • The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the specialty.
  • To facilitate the provision of high-quality specialist care to patients, treatment and follow up care in the vascular Virtual ward to outpatient setting.
  • Liaise with and refer patients to community nursing teams for Vac therapy and manage the patient journey on the virtual ward, clinic appointments and then discharge to community team once all equipment is in place.
  • Provide accessibility and troubleshooting advice for patients once discharged on the vascular virtual ward pathway by carrying a contact phone (in working hours).
  • Provide liaison with industry partners (3M) for staff training, consumable levels and educational opportunities.
