Job summary
We're looking for a Nurse Specialist to join our Regional Structural Heart Valve Team based at The Essex Cardiothoracic Centre (CTC).
CTC is a regional tertiary centre providing a specialist service for the comprehensive assessment, management, and treatment of patients with structural heart disease; including advanced structural heart interventions such as Transcatheter Aortic Valve Implantation (TAVI).
If you join our team, you'll be primarily responsible for the assessment, coordination, management, and ongoing support of patients referred to CTC for assessment, investigation, or treatment of structural heart disease, with a particular focus on aortic stenosis.
A key component of the role will be the day-to-day running of the Fast-Track Aortic Stenosis service, including triage of patients referred into the pathway, coordination of onward assessment and treatment, and support of patients across the regional network.
The role will also include reviewing inpatients at referring sites as part of the ongoing inpatient care of patients with aortic stenosis. Reasonable travel undertaken as part of these duties will be reimbursed in accordance with Trust policy.
Mentorship and support will be provided within the role, with the opportunity to develop knowledge and skills in pathway evaluation, service development, and data collection.So, if this sounds like the role for you please apply today.
Main duties of the job
If successful, some of your duties will include the following:
- Ensure that patients and their families are fully supported by providing clinical, psychological, and social support.
- Provide expert clinical advice to medical and nursing teams.
- Clearly communicate autonomous decision-making to medical and nursing teams.
- Prioritise care delivery within the ward setting, acknowledging the impact of decisions on other departments.
- Support the day-to-day operational running of the Fast-Track Aortic Stenosis service, including triage of referrals, prioritisation of patients, and coordination of appropriate onward review and investigation.
- Work as an autonomous practitioner and effectively deliver expert evidence-based care within the specialist area, managing a caseload of patients.
- Facilitate safe and timely discharge of patients with coordinated and evidence-based follow-up, ensuring appropriate onward referrals
- Contribute to data collection and service evaluation as part of the regional pathway review
