Home Therapies Nurse - Join Our Renal Team

We are seeking an enthusiastic, confident, and experienced Renal Nurse to join our dedicated team as a Band 6 Home Therapies Nurse. This is an exciting opportunity for a highly skilled practitioner with broad experience in both peritoneal dialysis (PD) and haemodialysis (HD) who is passionate about supporting patients to manage their dialysis safely and independently at home.

In this specialist role, you will draw on your comprehensive renal knowledge to assess, train, and coordinate both new and established home therapy patients. You will deliver excellent nursing care while providing personalised education to patients and carers, empowering them to feel confident in their chosen dialysis modality.

Working closely with the wider renal multidisciplinary team, you will also support the renal ward by sharing clinical expertise and contributing to staff training. Your work will include caring for patients within the Advanced Kidney Care pathway--offering education, supporting modality choice, and participating in group sessions, clinics, and one-to-one reviews.

This is a dynamic role that involves patient contact in both hospital and home settings. As such, a UK driving licence and access to a car are essential.

Main duties of the job

To develop the renal unit self-care Haemodialysis.

To work with the home therapies manager, the home therapies service, increasing the number of patients' dialysis at home in both peritoneal dialysis and home haemodialysis.

To help manage all home therapies patients at home, from prescription to line changes, managing the patients dialysis requirements

Work with patients ensuring dialysis at home is safe, and ensure all homes and water are within recommended standards.

To act as an accountable and autonomous specialist nurse, to complement the home therapies services.

Provide specialist clinical knowledge and collaborate with professional colleagues in renal, medical, surgical, interventions radiology and community settings.

Assess, develop and implement specialist nursing care plans and provide safe care, advice e support and provide education to patients and staff.

Participate in research and lead on clinical audit in relation to speciality to influence future practice.

Produce reports regularly about patient satisfaction and dialysis adequacy.

Provide clinical and professional support for colleagues and junior staff in the team and across the the wider renal services.

Escalate concerns of deterioration in patient's condition effectively.

Act as the patients advocate.
