Job summary

An exciting opportunity to work within a developing and expanding team has arisen. The Discharge to Assess (D2A) team is a team of Therapists who work with people within their homes on discharge from Hospital. This is to ensure patients needs are being met at home and provide an opportunity for more accurate assessment in their own homes, compared to the unfamiliar setting of an acute hospital

The Therapist (Occupational Therapist or Physiotherapist) will need to work holistically within the MDT and utilise their profession specific skill-set to deliver high quality care and support the patient in their own home.

Experience of working in the acute or a community setting will be helpful. The role will involve working with multiple agencies, inside and outside of the hospital, to provide the best service for the people in our care.

MSE has a focus on developing the clinical and non-clinical skills of its staff both personally and professionally.

Successful candidates will need to establish and maintain excellent communication with a variety of service users and staff groups, assessment and formulate treatment plans and work within a variety of clinical and non-clinical environments. They will be professionally and legally accountable for their caseload and documentation.

Due to the nature of the role, you will be required to hold a valid UK driving licence and access to their own mode of transport as frequent travel in require for this role.

Main duties of the job

Be responsible for undertaking assessment in the community for patients under the Discharge to Assess (D2A) pathway. Assessment to identify a patient's therapy needs and provide intervention to promote safety, independence and reduce the risk of re-admission or harm.

Coordinate services by undertaking a person-centered assessment; developing, implementing and evaluating care plans, assessing equipment needs and potentially working on rehabilitation goals as service capacity allows.

Work autonomously within professional guidelines and the overall framework of the service's policies and procedures.

The team is a multi-skilled team and all staff will receive training to be multi-skilled to provide comprehensive assessments and treatment of patients.

You will undertake the learning and the implementation of new specialist skills and knowledge to include some physiotherapy skills or Occupational therapy skills. This may also include assessing for and requesting health or social care packages . All these skills are competency led, and appropriate training will be provided where needed.
