An exciting opportunity for a Staff Nurse Secondment/fixed term contract for up to one year 22.5 hours (3 days) has arisen within Cardiac Rehab at Southend Hospital.
We are a small, highly experienced , forward thinking, friendly, Multi disciplinary team who work with patients post discharge from hospital following admissions with ACS/PCI/CABG/Valve Surgery.
You will have responsibility for the assessment and management of patients with Coronary Heart Disease and other Cardiac conditions, who are referred for Cardiac Rehabilitation. Providing an individualised health education programme, post hospital discharge. Assisting patients in managing their condition and adopting a healthier lifestyle by addressing identified risk factors.
You will contribute to the development and progression of the CR service.
You will adhere to the NMC Code of Practice at all times and ensure compliance with the application of hospital policies and procedures.
Main duties of the job
Assess and provide health education and support for patients referred to CR from Southend hospital, Essex CTC and other sources.
Review ECG and test results and action as appropriate. Refer patients, as required, to other members of the MDT.
Participate in the assessment and core rehab as required, either by telephone or face to face.
Risk stratify and assess patient's suitability for inclusion on the programme and maintain regular contact with the patient whilst on the programme either by telephone, virtually or face to face.
Liaise with ward staff, departmental staff, patients, and relatives concerning specific patient needs and deal with these effectively, offering psychological support and any appropriate resources/information.
Liaise as necessary with other members of the multi-disciplinary team as required, e.g. CNS, Cardiologist, dietician, counsellor etc.
Encourage the concept of providing continual Health Education to cardiac patients whilst in the hospital setting and post-discharge.
Work with Cardiology Ward Staff in educating staff and implementing the Health Education programme to patients with acute coronary syndrome and their carers.
Review and help develop relevant health education literature as necessary.
Liaise with 'Take Heart' cardiac support group and signpost patients to the self-help group as required.
