Cardiac Physiology Specialist Patient Pathway Coordinator

Job summary

Due to the previous longstanding postholder relocating, this friendly, professional team are inviting applications to our Specialist Patient Pathway Coordinator post. The role is central to the efficient and safe delivery of our Cardiac Physiology services which span echo, pacemaker, ICD , ECG monitoring and cath lab services for the Essex region and our local Basildon District General Hospital . This is a busy , interesting, varied and fulfilling role providing administrative support to the Cardiac Physiology Management team with specific tasks relating to the follow up of patients with implanted cardiac devices. We are seeking a new gem! - flexible, professional and dynamic . Experience in a Cardiac Physiology administrative role is preferred but applications are welcomed from experienced NHS patient service administrators with transferable skills. Support will be in place to learn new systems or tasks.

Main duties of the job

About the Role

Sole specialist patient pathway coordinator for the CTC Cardiac Physiology team responsible for facilitating the smooth running of all patient- centered administrative processes. The scope of services include Echocardiography, Cardiac Rhythm Management and Primary Investigations. The postholder will work closely with the Professional Manager, Heads of Service and includes supervision of a team of Patient Pathway Officers in our dedicated booking office.

Whilst the postholder will oversee the efficiency and standards of booking office processes, they are responsible for carrying out key specific administrative tasks relating to the follow up service for patients with implanted cardiac devices.

Provide general administrative support to the Cardiac Physiology management and clinical team ensuring effective co-ordination of diaries, staffing rotas, resource availabilities and stock levels.

MAIN DUTIES & RESPONSIBILITIES

Act as first point of contact on behalf of the Cardiac Physiology management team for enquiries made by telephone calls, emails or personal callers to the general office. Respond efficiently and promptly, redirecting as appropriate.

Support service managers with administrative tasks using MS Office software with particular requirement of expertise in Excel.

Coordinate, monitor and initiate appropriate changes to admin processes for all Cardiac Physiology services to ensure the patients journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.

Set up, review and adjust relevant system booking templates and sessions e.g. CVIS and EPR, in liaison with appropriate service leads to ensure they meet the demands of the service.

Monitor compliance with contractual access targets e.g. DMO1 6 week diagnostic waits, 18 weeks Referral to Treatment target (RTT) escalating concerns to relevant Cardiac Physiology and divisional managers.

Monitor compliance with local clinical scheduling standards e.g. 2-6 week post implant device checks, planned / surveillance diagnostics escalating concerns to relevant Cardiac Physiology and divisional managers.

Where practicable, support the maintenance of an effective booking office by providing backfill cover for Cardiac Physiology Patient Pathway Officers, ensuring own specific duties are not compromised.

Tasks and responsibilities specific to device follow up service:

Lead point of contact for telephone enquiries from patients under the care of the Cardiac Physiology cardiac rhythm management (CRM) team for the technical follow up of their implanted cardiac devices e.g. pacemakers and defibrillators.

Following local protocols disseminate to Physiology device team or directly action as appropriate depending on context clinical, technical appointment related.

In accordance to local CRM service protocols and within scope of knowledge, directly provide advice and guidance in response to non-clinical FAQs e.g. home monitor technical troubleshooting, lifestyle queries, device manufacturer support line contact details.

When necessary, following initial review of queries by either Physiologists or company representatives, pass on specific advice to callers.

Provide guidance to patients requiring urgent or emergency clinical attention e.g. shock delivered, chest pain, difficulty in breathing.

Responsible for arranging the first appointment for patients following their cardiac device implant within appropriate timescales, including creating Device ID Card and dispatching to patients

In liaison with service leads make amendments to device clinic templates adjusting appointment slot types and session availabilities to cope with demand.
