Main duties of the job
We are looking for an enthusiastic and passionate individual to join our CNS Stoma team at Southend hospital.
The post holder will promote and develop high standards of care for Uro-oncology patients.
You will work in close collaboration with the colorectal services and other members of the multi-disciplinary team to ensure a safe and effective pathway of care.
Support of patients and their family is an essential part of this role, along with sharing CNS expertise with other members of the multi-disciplinary team.
This role is ideal for any nurse with either colorectal or urological surgery experience who wishes to further develop into the stoma nurse CNS role.

MAIN DUTIES & RESPONSIBILITIES
Be able to plan the appropriate care pathway to meet the needs of the individual patient in both hospital and home settings.
Understanding the full range of treatment options available to each patient and have the ability to discuss these with the patient and family
Be able to share and demonstrate knowledge and understanding of normal and abbnormal gastrointestinal patho-physiology
Have a clear understanding of the range and purpose of GI investigations and the associate needs of the patient.
Demonstrate a clear understan ding of the principles of the enhanced recoverry programme and their application to clinical practice.
Has the ability to generate a differential diagnosis and plan, to implement and evaluate individual treatment/interventions and care packages for a selection of patient groups and show evolution of breadth of skills
Demonstrate to ability to answer relevant questions from patients and their families about thr patients condition, and supply solutions where possible.
Support the maintenance of the patient support group and bewsletters abd facilitate as apporpirate
Maintain Nurse led clinics in both hospital and community settingsC.
Clinically examine and assess patient needs form a physiological and psychological perspective and plan clinical care accordingly, advising patient carers and colleagues
Work with patients in order to support compliance with and adherence to prescribed treatments
Ensures the use of evidence based care in the delivery of care
Priorities health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
Work effectively with others to clearly define values, direction and policies impacting upon care delivery.
