An exciting opportunity has arisen with the Neurology team at Mid and South Essex NHS Foundation Trust.
We are looking for a passionate health care professional to join our new team supporting patients with Parkinson's diagnosis across our Trust's geographical area. The successful candidate will be a registered nurse, with previous experience working with complex neurological conditions.
Main duties of the job
Your role will be to provide clinical case management for patients with Parkinson's, both in the acute wards and in out-patient clinics.
You will work as an integral part of the multi-disciplinary Neurology Teams Mid and South Essex and alongside other relevant services, such as local hospices, and across care providers in all areas, to ensure continuity of care when patients are referred on to and from these services.
You will lead on the development and monitoring of care plans, alongside the community care plans to ensure continuity of care across providers, as recommended in the National Service Framework (NSF) for Long-Term Conditions.

- Supporting others (mentor / SSSA supervisor) experience
