Job summary

Provide specialist clinical Pharmacy services and advice to a specified clinical speciality(s), demonstrating a high level of clinical effectiveness. Take part in the provision of general clinical, dispensary and pharmacy on call services.

Main duties of the job

  1. Communication

    • To develop and maintain effective working relationships, which inspire confidence in clinical pharmacy and the pharmacy department, with staff and patients in all rotation specialties
    • Work effectively as a member of the multidisciplinary healthcare team
    • To inform and educate patients, carers and members of the public about the safe use of medicines.

    Analytical and Planning

    • Be responsible for planning and organising own workload in accordance with the requirements of the rotation specialty

Job responsibilities

Clinical Duties

  • Provide a Clinical Pharmacy Service to a specialty. This will include key aspects of medicines optimisation:
    • Reconciling medicines on admission
    • Screening of patients on admission for pharmaceutical care issues and formulation of care plans where pharmaceutical care issues are identified
    • Review medication to ensure safe and effective use
    • Review and update of care plans depending on response, results of investigations and adverse effects
    • Therapeutic drug monitoring for specific drugs
    • Educate patients about their drug therap
  • Attend clinical ward rounds, clinical and multidisciplinary meetings relevant to the specialty as appropriate.
  • Provide specialist pharmaceutical information to medical, nursing staff, patients and to General Practitioners and other staff within the Trust where appropriate.
  • Practice as an independent / supplementary prescriber in line with local legislative and clinical policies / guideline
  • Clinically screen and final check out patient and discharge prescriptions according to current practice
  • Resolve any prescription queries with the prescriber and amend prescription accordingly
  • Participate in dispensary duties e.g. prescription processing and dispensing, ensuring appropriate records e.g. controlled drug register are maintained, when required
  • Answer medicines information queries from medical, nursing staff and patients, using appropriate resources
  • Promote the safe, appropriate and economic use of drugs

Staff Management

Supervise and train junior members of staff in Dispensary and Clinical ward duties.

Participate in the recruitment of staff when required.

Participate in systems for monitoring performance competencies, audit and staff appraisals.

Policy and Service Development

Develop and implement new protocols and procedures for area of work, and assist in development of pharmaceutical aspects of policies for this area.

Plan own workload within allocated area of work.

Undertake research and audit projects in area of speciality and contribute to ongoing audit and ad hoc research projects elsewhere as required.

Financial and Physical Resources

Promote the safe, appropriate and economic use of drugs.

Keep accurate patient medication and drug issue records using the Pharmacy JAC computer system.

Generate reports/graphs/presentations using appropriate computer software when required.

Monitor drug expenditure within speciality, produce budget statements and drug usage reports, interpret and provide analysis and recommendations.

Manage the cost effective use of medicines including high cost drugs e.g. biologics.

Take payment for prescription charges, issue receipts and refunds according to departmental procedures.

Ensure the safe and secure storage of Pharmacy stock of drugs and other materials.

Monitor the safe and secure storage of drugs on wards.

Ensure all equipment has been regularly maintained.

Ensure all equipment is used in accordance with the manufacturers instructions.

Ensure any equipment found to be faulty/unsafe should be removed from use and reported to the appropriate authority.

Information Resources

Answer medicines information queries from medical, nursing staff and patients, using appropriate resources.

Provide specialist counselling and advice on the use of medicines to patients, medical and nursing staff.
