Patient Pathway Co-ordinator - Cardiac Rehab

Job summary
The post holder will be solely responsible for the day-to-day administration for the Cardiac Rehab department. This will involve taking responsibility of processing referrals and ensuring the accurate booking and recording of patient data.
You will be required to co-ordinate patient details and keep up to date information on the status of these patients whilst maintaining patient confidence. The post holder will also be required to provide other administrative support as required to meet the demands of the service.
Applicants should have at least 3 years admin experience with knowledge of the 18 week Referral to Treatment processes, enhanced computer skills, have the ability to work in a multidisciplinary team setting and an eye for detail to ensure all systems are up to date and relevant.
Main duties of the job
- Inputting all relevant information and statistics onto the NACR (National Audit of Cardiac Rehab).
- Retrieving various statistics from the NACR data base as required.
- Keeping manual records of activity and outcomes as requested.
- Updating the team regularly on the waiting times of all rehab clinics and assessments.
- Ensuring all referrals are accurately recorded on the data base and scanned electronically to their local Cardiac Rehab within the specified time frame.
- Booking patients into clinics and assessments as required.
- Answering phone calls and emails and transferring to the appropriate professional.
