Outreach Critical Care Nurse

Job summary

We have a fantastic opportunity to join Colne Ward as an Outreach Critical Care Nurse. Colne ward is a Cardiothoracic Critical Care Unit within the Essex Cardiothoracic Centre based at Basildon Hospital that cares for patients following Cardiothoracic surgery as well as those undergoing cardiology procedures.

Colne ward has a multidisciplinary team approach to caring for our patients which includes nurses, medical staff, physiotherapists, dietitians and HCA's. The team encompasses diverse, multicultural individuals which work throughout the centre.

Your role will be to undertake direct clinical involvement with Level 1, 2 & 3 patients both on the ward and following admission/discharge from the Critical Care Unit allowing dissemination of expertise and practice skills, as well as acting as a clinical role model.

Main duties of the job

Your role will be to assist with the development of promoting protocols for recognition and management of critically ill patients throughout the centre and the development of standards and (where appropriate) care pathways for patients requiring critical care in a ward environment and in Critical Care Unit

You will assist with the development of monitoring discharge planning for patients transferred from Critical Care Unit to the wards, working with all disciplines, to educate, develop and support understanding of the needs and management of critically ill patients and the Critical Care Unit /Education staff in developing skills and knowledge required to care for acutely ill patients in the ward environment.
