Hospital@Home service is looking to recruit 3 enthusiastic and experienced registered nurses or Registered Allied Healthcare Professionals to join their team based at Broomfield Hospital

The post holder will work as part of the Hospital at Home team as a Community Senior Nurse/AHP primarily within the community setting for the Mid and South Essex Hospitals; supporting the team, to manage the effective delivery of services to patients in their own homes across the Mid Essex NHS Trust areas.

This is a highly visible role providing strong professional and clinical leadership within the Hospital@Home (H@H) team. The successful candidate will assist with supporting the sub-acutely unwell patient by providing care in the patients home; administering IV medications, wound care management, supporting admission avoidance, triage and management of the patient in the community setting.

A driving license and access to a car are essential for this role as you will be expected to travel between patients' homes and their hospital base site.

Main duties of the job

If you enjoy holistic one to one care, with the time to really support your patients, then this is the job for you.

The role is predominantly based in the community, therefore you will need to have a full driving licence and have access to a vehicle that can be used for work purposes.

You must be a registered healthcare professional as you will be administering Intravenous medications.

Main duties to include but not limited to:

  • Administration of IV medications in the patients home via Peripheral cannula or PICC line.
  • Wound management
  • Supporting admission avoidance
  • Assessment and review of treatments
  • Support patients through their treatment via agreed clinical pathways
  • Review care plans and escalate concerns appropriately
  • Deputise for the virtual ward manager in their absence
  • Any other duties associated with the Hospital @ Home service.

Hospital@Home operates across all three sites within the Mid & South Essex University Hospitals Group and you will need to have a driving license and access to a car in order to perform this role. Travel expenses are paid.
