Job summary

Katherine Monk ward is the acute General-Surgery ward at Basildon Hospital. The ward is an ever-changing, fast-paced area to work, so if you are an enthusiastic individual that enjoys working as part of a team then please apply.

You must have a working knowledge of the trust statutory procedures for discharge planning and the procedures for the transfer of care policy and will under the supervision, direction and guidance of qualified nursing staff and ward manager, initiate, assess and implement discharge planning for patients from admission and coordinating this throughout their stay in hospital.

You will be supported with the appropriate competency based training for the clinical elements of the role to include removal of cannulas, vene-puncture and decontamination of bed areas.

Main duties of the job

In your role you will liaise with other members of the MDT and the patient's family and carers in order to facilitate efficient and timely discharges, supporting the nursing staff of the ward to ensure speedy discharges are achieved through a combination of the administrative and clinical elements of the role as described below.

You will be the focal person within the ward environment to ensure that the patient and their families are informed throughout the discharge process to ensure a seamless journey and identify, in a timely manner, any onward service requirements and will be pivotal in liaising with the family's, social services care providers and other members of the MDT to co-ordinate assessments prior to discharge.

You will have current Information Technology skills to enable the input of data on electronic tracking systems such as 'teletracking' and will display exemplar admiration capability.
