Job summary

Our Cardiology Heart failure Service has opportunities for 2 Clinical Nurse Specialists to join its exciting, supportive Team either on a full time or part time basis.

In your role you will carry continuing responsibility for the provision of a specialist service, which provides high standards of evidence based care to patients admitted with a cardiac diagnosis and help reduce length of stay in hospital by ensuring patients follow the correct pathway of care.

Your role is a nurse led focus on in- patients with AF/HF diagnosis including the development of clinical practice, developing and delivering national and local priorities, education and training, and audit.

You will be empowered to make high level clinical decisions and be responsible for providing clinical advice and support to, and liaising collaboratively, with all areas of the multi-disciplinary team, together with patients and their families.

Main duties of the job

Your role is to promote excellent practice trust-wide in the assessment, diagnosis and management of cardiac patients using agreed policies and guidelines/pathways of care and acting as a liaison between the Acute Medical Team, Cardiologists, Nurses and MDT ensuring timely review of cardiac patients, especially those with HF, including advice and requesting of specific tests and appointments.

You will practice autonomously as a core member of the cardiac team and wider multi-disciplinary team, continually evaluating the quality of patient care and be one of the lead nurses for those patients with a diagnosis of HF.
