An exciting opportunity has arisen for a Junior Charge Nurse/Charge Nurse to join our team. We are a nurse led unit, caring for patients who are medically fit for discharge but not quite ready for home. Our goal is to discharge patients safely back into the community.
The Active Home Unit is a nurse led unit which is made up of the discharge lounge as well. We care for patients who are medically fit for discharge but may require other factors in order to aid their discharge.
Main duties of the job
As a charge nurse / junior charge nurse your role is to take charge of shifts and ensure that junior staff are well supported and ensure that the flow of the shift runs well.
Patient safety is key and your job will involve excellent communication between all members of the multi-disciplinary team to ensure that patients are cared for effectively and holistically.
You will have a sound clinical knowledge of the unique clinical needs of medical patients and a desire to deliver the right treatment in the right place at the right time.
In addition to your clinical expertise you will have an interest in developing your leadership style, people management expertise in conjunction with the HR team along with excellent inspiring communication skills.
You will be committed to leading by example, encouraging and empowering your team along with all members of the MDT to ensure consistently high standards of care are set and delivered.
This role will operate within the clinical governance framework of the organisation and will work in a collaborative approach with the multidisciplinary team.
The registered nurse within the Active Home Unit is responsible for the safe administration of medications and other treatments provided adhering to Trust protocols and policies.
To aid safe discharges of patients to their appropriate destinations, ensuring all checks have been carried out and information has been passed on.
