Job summary
This is an exciting opportunity to join our well established stroke team at Southend hospital. The post holder is a pivotal role in the co-ordination of the stroke thrombolysis team to provide assessment and thrombolysis services to the southend site. The role works closely with the departmental senior sister/charge nurse to manage capacity within the unit and triage the stroke patients effectively to the unit in line with our SNAPP targets. This is a blend of clinical and managerial working and provides a conduit between primary, secondary and tertiary care to provide high standards of care to our patients.
Main duties of the job
- To facilitate the daily running of the acute stroke unit whilst maintaining patient safety and effective flow.
- Assist in the development, implementation and monitoring of Governance systems in the ward/department relating to Risk Management, Health and Safety and Controls Assurance.
- Promote and develop the use of evidence-based practice to improve the outcomes of patient care within the acute stroke pathway. Ensure all Trust Visions of care standards are adhered to.
- Teach on pre and post registration programmes for health professionals liaising closely with local education providers and the Trust's Learning and Development Team.
- Work closely with the Stroke unit manager to develop and provide stroke care.
- Convey complex and sensitive information (e.g. abnormal test results) in a comprehensible and empathic manner.
- Be first line of contact for patients, relatives, GP's and practice nurses for reassurance, expect advice and discussion of treatment plans.
- Convey and present specialist information, written or verbal, formally or informally, as appropriate. Report appropriate clinical information to senior staff. Ensure that written documentation is kept in respect of all investigations and incidents.
- Provide training and education to stroke unit staff formally and informally.
- Co-ordinate data collection to produce evidence of clinical effectiveness in order to meet the National Stroke Strategy, NSF and Royal College of Physicians guidelines.
