Job summary

Our Elderly Care Medicine service has 2 exciting opportunities for Consultants to join our dedicated, experienced and patient focused team caring for our older patients across our Acute wards .

We have two roles both offering liaison opportunities . One within surgical and the other Ortho-geriatric cross cover .

Both roles involve working within our frailty unit and being part of a multidisciplinary team providing high quality care to patients within our community.

We are happy for either of these posts to be offer as Substantive post or as a Locum Post with a view to becoming permanent depending on your circumstances.

Main duties of the job

As our Consultant you will be responsible for the delivery of a dedicated programme of geriatric care, assume responsibility for patients on Elderly Medicine wards with daily review and full consultant ward rounds, regularly pro-actively review of our patients and coordination of care with allied specialties and engage in multi-disciplinary meetings to direct the care of patients.

You will also provide specialist opinions to other hospital specialties as part of frailty liaison service, participate in outpatient clinics supporting the delivery of patient care within Geriatric Medicine in line with national standards and Care Quality Commission requirements, actively participate in developing innovative approach to acute elderly care, develop older peoples acute service pathways, services and Outpatient clinic to facilitate the further development of our Frailty Assessment Unit and actively participate in clinical audit and quality improvement initiatives.
