Job summary

An exciting opportunity has arisen within the Cardiology Department at Basildon Hospital, for a part time Patient Pathway Co-ordinator to join our friendly Secondary Care Cardiology Team.

As a Patient Pathway Co-Ordinator you will be responsible for co-ordination of booking Cardiology clinics and managing/monitoring the patients journey from referral to treatment.

Applicants should have at least 3 years admin experience with knowledge of the 18 week Referral to Treatment processes, enhanced computer skills, have the ability to work in a multidisciplinary team setting and an eye for detail, to ensure all systems are up to date and relevant.

As a Patient Pathway Co-Ordinator you will provide administrative support to all clinical staff within the clinical area including; consultants, Nurse Specialists and Junior Doctors ensuring effective co-ordination of their diaries etc. including cancellation and reinstating of clinics in line with planned and unplanned leave.

As well as supporting the clinical team and other Divisional Managers in ensuring the specialty are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets. To ensure a consistent and effective administration service is maintained at all times.

Supervise Patient Pathway Assistants within the relevant specialties including: attendance management, appraisals and general monitoring of performance and compliance with Trust, Divisional and Specialty standards.

Main duties of the job

Your role involves co-ordinating, monitoring and initiating appropriate changes within administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales and ensure pathways are validated throughout the patient journey and resolve/escalate concerns within the pathways accordingly e.g. risks to access targets etc.

You will be required to utilise computerised tracking systems to locate case notes and investigation results, liaise with teams outside of the speciality e.g. GPs where appropriate to ensure a seamless patient journey, monitor work lists for Choose and Book to monitor Consultant workflow and escalate to Operational Managers if necessary and monitor patient attendance to appointments and re-arrange/book further appointments as required.

You will set up, review and adjust clinics in liaison with appropriate clinicians, provide PA duties for Consultants including maintenance of diaries, appointments, co-ordination of leave, co-ordination of expense forms, correspondence derived from any educational/supervisory or research activity. Ensure regular communication with medical staffing team in all aspects of leave and will take minutes at relevant MDT and Audit meetings.
