The post-holder will be responsible for the management of a defined caseload (patients/service-users) and will provide specialist nursing support, advice, education and/or care to that specific group. They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards for respiratory patients.
Main duties of the job
Carries continuous responsibility for a defined caseload (patients/service-users) including setting standards, assessment of needs and the development, implementation and evaluation of programmes of care following (clinical) protocols within respiratory. Organises, prioritises and evaluates workload based on accepted referral and discharge criteria in line with operational protocols. Responsible for ensuring own practice is evidence-based.
Assesses, plans, implements and evaluates specialist care or treatment regimens for respiratory patients, taking into consideration the lifestyle, gender, cultural background and preferences of the individual, making reasonable adjustments to care where this is appropriate.
Works as part of the MDT fostering effective professional relationships to ensure effective communication and a seamless service for a defined caseload (patients/service-users).
Works across organisational and professional boundaries as an expert practitioner as part of the multi-disciplinary team e.g. liaising with colleagues in primary care (District Nurses, GPs etc.).
Promotes health education and health promotion with patients and carers, ensuring that they are involved in decision making.
Utilises interpersonal and communication skills in order to convey complex and sensitive, and potentially contentious information with empathy and reassurance, overcoming barriers to understanding where necessary.
