This is an exciting opportunity for a 12 month secondment to join the hospital specialist palliative care team, and work as part of a dynamic, proactive and supportive team. The post holder will work as part of the Multi-Disciplinary Specialist Palliative Care Team providing palliative care to enhance the quality of life for patients / carers with any life limiting illness requiring palliative care, including complex physical, psychological and emotional needs. The post holder will also be involved in the delivery of End of Life education for staff. There will also be the opportunity for personal & professional development following a Core Competency Framework.
Main duties of the job
The post-holder will be responsible for the management of a defined caseload (patients/service-users) and will provide specialist nursing support, advice, education and/or care to that specific group. They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within the Palliative Care Team.
