Following the exciting development of our Multispecialty Virtual Ward, we have a number of opportunities for roles within the service.
The Virtual Ward is an innovative service and the aim to is support people to receive care, support and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.
Due to current home office guidelines, Bradford Teaching Hospitals NHS Foundation Trust is unable to provide sponsorship as part of this role.
Main duties of the job
This is an opportunity to gain experience and shape the development of the new service for the future. The role will involve working as part of a multidisciplinary team both within secondary care and the community providing care at home.
The successful applicant will provide support to the nursing team in providing nursing programmes to people being managed on virtual services pathways in the community. The post is an integral part of the multidisciplinary team which contributes to supporting patients to meet their nursing goals in their own home following an admission to hospital or a period of ill health. A full Driving license is essential for this role.
