The Cardiac Rehabilitation service has 2 exciting vacancies for two full time Band 5 Nurses. Both are 12 month Fixed Term Contract posts . This post is a community role within a specialist Nursing Team.
Main duties of the job
Assisting the community cardiac rehab nurses with caseload management , in clinics telephone & home visit patient contact . Running exercise classes and seeing patients on the wards

- Post registration experience in cardiac, acute medicine or community nursing.
- Experience in working with vulnerable groups
Desirable
- Heart failure care experience
Skills

- Car driver / owner with vehicle to use for work
- Ability to work autonomously
- Excellent communication skills
- Clinical assessment skills
- Ability to support more junior members of the team and students
- Must be computer literate
- Needs to have an awareness of quality issues, audit and evaluation
- Evidence of ongoing professional development
- Must be willing to undertake further training and education as required.
