An exciting opportunity has arisen for a Band 4 support worker to work alongside the continence nurse specialists

The continence team offers a friendly and supportive environment and we are looking for a highly motivated and forward thinking individual. The key part of our service is to provide a high standard of quality holistic continence assessments and treatments to maximise independence and wellbeing to ensure that the ever changing health needs of a patient are met within a very busy but extremely rewarding service.

The role will consist of the day to day clinical support for the continence team and will also include ongoing review of continence products and assessment and support around complex toilet training etc. There will be an expectation of some flexibility as the role does involve working alongside the wider multi professional team within a variety of settings across Bradford and Airedale .

The candidate will be required to work in both hospital and community settings and deliver nurse led clinics, across the Bradford and Airedale district.

A driving licence and access to a motor vehicle in order to carry out duties is essential (reasonable adjustments will be considered).

Main duties of the job

  • Undertake the ongoing assessment and monitoring of children requiring continence products & identify those who may benefit from toilet training. Running independent clinics when confiden
  • Organise continence product supplies & ensure families/carers are trained in their use ensuring appropriate continence products are provided
  • Devise, implement & monitor toilet training programmes in conjunction with parents /carers & the wider MDT as appropriate
  • Undertake follow up clinics to monitor children, as required
  • To encourage families to maximise independence by promoting resources availabl
  • To assist with the management of a caseload of children with continence needs, assessing, planning, implementing & evaluating their individual programme of care, taking advice from clinical lead when required
  • Identify child/parent learning need & modify approach accordingly. Develop strategies & tools for learning
  • Undertake assessments of development skills around toilet training providing advice to parents/carers/professional
  • Identify risks / clinical incidents and act in accordance with local policy
  • Ensure a safe environment during interventions with all children/young people, families & colleagues
  • Facilitate group sessions as required
  • Update specialist continence knowledge to ensure your area of specialty is evidence based & in line with NICE/trust guidelines.
