The purpose of this exciting new role is to support the Breast Cancer Team to implement the early diagnostic and treatment pathway for patients with Breast cancer to achieve the 62-day cancer pathway. This role has been specially designed to support the optimal breast cancer timed pathway. The post holder will be involved from onset of the patients care pathway and will require to evolve to support the pathway as it continues to refine over time.o To collaborate and communicate with the CNS and wider Health Care Team/MDT to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.o The post holder under supervision of the Clinical Nurse Specialists & MDT lead, deliver patient-centred, support as necessary to non-complex patients and proactively identify and address obstacles within the patient care pathway. This includes maintaining open communication with patients, coordinating with healthcare professionals, and ensuring timely completion and expediting diagnostic test appointments aligned with national and trust guidelines
Main duties of the job
To support the Breast Cancer CNS team and multidisciplinary team in delivering a highstandard support to patients with cancer. This will involve being in regular contact with patientssuffering from a range of breast cancers.The post holder will be expected to support and signpost patients over the telephone andprogress their care pathway in liaison with other colleagues and departments in the hospital.The post holder will also be expected to be involved with audit, the WY&H Cancer Alliance andthe Peer Review processes.The post holder will work in close liaison with primary care, Multidisciplinary team members andpatients supporting cancer pathways, by providing logistical support to ensure tests areundertaken in a timely manner.The post holder will be responsible to liaise with the MDT Co-ordinator and working closely withthe Management and Clinical teams, the post holder will be responsible for ensuring thatdeadlines are met and that there is a pro- active co-ordination of cancer patient pathways toensure appropriate treatment is received in line with national and local targets.
