Job summary
An exciting opportunity for a qualified and highly motivated registered nurse to join the expanding Elderly virtual ward team to deliver frailty in reach into A+E and to support the development of a front door frailty same day assessment unit.
The successful post holder will work as a dynamic and proactive specialist nurse within our very successful and developing Integrated Frailty service. The service operates over 7 days and provides the frail population of Bradford the benefit of a holistic Comprehensive Geriatric Assessment.
You will be based at St Lukes Hospital, leading a team delivering urgent care response, same day care and discharge to assess.
As a team member you will be part of a friendly, supportive, and professional integrated team which comprises of Geriatricians, Nurses, Occupational Therapists, Physiotherapist, Co-ordinators, Advanced Clinical Practitioners, Pharmacists, Social services, and Community services.

Main duties of the job
The post holder will be responsible for leading and managing a clinical team day to day, delegating work to staff, support the development of nurse/therapy led initiatives and developing seamless and integrated care pathways between acute wards and primary care, in liaison with clinicians in partner organisations.
The post is an integral part of the multidisciplinary team, responsible for the delivery of specialised nursing/therapy skills within the interface of virtual ward/front door frailty. The post holder is accountable for their own actions without direct supervision overseeing a complex caseload (clinical time 60%) The post holder will act as a role model for staff within the EVW/FDF service.
Responsible for staff management and supervisory responsibility for Nursing/therapy, support, and administration staff within the EVW/FSF service.
