Job summary

Do you want a new challenge ? An exciting opportunity has arisen to work as a Personalised Care Lead within the Lead Cancer Team.

We are seeking a motivated nurse nurse with a passion and commitment to improve the cancer pathway and experience of patients living with and beyond a cancer diagnosis.

The post holder will work closely with the Lead Cancer Nurse to deliver on personalised care projects, identified in the NHS Long Term Plan.

Main duties of the job

To embed, and improve personalised care interventions including:

  • Holistic Needs Assessments (eHNA)
  • Personalised Care Support Planning (PCSP), based on HNA
  • Health & Wellbeing Information & Support (HWIS)
  • End of Treatment Summaries (EOT)
  • Personalised Stratified Follow Up & Self Supported Management (PSFU/SSM)
  • To implement the ACCEND programme within the nursing teams.
  • To participate in the engagement of the pre habilitation programme for cancer patients.
  • To engage with patients and patient groups providing valuable patient feedback, to drive service improvement.
