An exciting opportunity has arisen for experienced nurses to become part of the COPD/ Asthma Nurse Specialist Team at Bradford Teaching Hospitals.
The team of COPD/ Asthma Nurse Specialists will provide a 7 day service covering from 8am - 4pm Sunday - Monday ensuring all patients have a specialist inpatient review within 24 hours of admission. The COPD / Asthma Nurse Specialist will be responsible for reviewing identified COPD / Asthma patients within timeframe and completion of COPD/ Asthma discharge care bundle
Summary of most important Experience/Skills/Qualifications required:
RGN
Must have teaching qualification ie SLIP / ENB 998
Have experience in acute medicine and caring for patients with COPD/ Asthma .
Excellent communication and interpersonal skills
Ability to manage time and workload
Capable of multidisciplinary team work
Excellent leadership skills
Will need to be flexible with shifts, working 8am-4pm and will need to be able to work weekends.
Main duties of the job
The COPD/ Asthma Nurse Specialist will be responsible for:
- Ensuring patients receive specialist input to their care within 24 hours of admission thus risk stratifying, investigating and ensuring appropriate management of COPD/ Asthma patients thus reducing unnecessary admissions, length of stay in hospital and improving patient flow and experience.
- Providing Trust wide and timely expert COPD/ Asthma CNS specialist review and advice on the management of acute exacerbation patients with COPD/ Asthma Trust wide. This will include checking inhaler technique and reviewing medication including oxygen and where required arranging home oxygen, writing self-management plans if appropriate, promoting smoking cessation, assessing for suitability for pulmonary rehab.
- Facilitating early discharge with completion of COPD Pathway / Discharge Care Bundle.
- Providing follow up (either by telephone or in person) within 72 hours of discharge.
- The COPD/ Asthma CNS team will cover 7 days a week to ensure all patients are reviewed timely and have a specialised patient specific management plan formulated
- Participation in the mandatory COPD audit.
The post holder will provide education to patients, carers, medical and nursing staff at Bradford Teaching Hospitals with emphasis on educating patients with regards to treatment, respiratory support and readmission prevention to promote patient flow.
