An exciting opportunity has arisen to join the Bradford Teaching Hospitals Foundation Trust diabetes service. Based with the innovative diabetes nursing service you will play a vital role in supporting and delivering the dynamic and evolving diabetes specialist nursing service, including an in-patient diabetes nursing service. We are fully integrated with the wider diabetes specialist team and benefit from ongoing supervision and governance support.
Main duties of the job
Brief Description of the Role:
Within this challenging and mobile role the post holder will work as part of an integrated diabetes multidisciplinary team providing various types of clinical support to community and hospital based clinics, healthcare professionals and directly to individuals with diabetes. This support will include clinic attendance, phone contact and home visits.
The post holder will also work as part of a nursing team, supporting the implementation of Think Glucose across Bradford Teaching Hospitals Foundation Trust ward and departments. This involves supporting ward staff in delivering diabetes care and working directly with patients and carers. Think Glucose is a national initiative designed to improve the care of in-patients with diabetes.
You will also be involved in the delivery of our locally developed structured patient education programmes and our various healthcare professional clinical education sessions.
