Job summary
An exciting opportunity has arisen for a highly motivated, innovative and committed band 6 Discharge Coordinator, to join our established team of discharge co-ordinators working within MAIDT.
The successful candidate will need to have excellent organisational and negotiation skills along with an excellent knowledge of the discharge processes for patients with complex needs along with an understanding of Continuing Care Criteria and a working knowledge of Safeguarding principles. Excellent communication skills are also essential to enable the individual to work sensitively with patients and families at a very difficult time. The ability to co-ordinate the many services which contribute to supporting a complex discharge is also essential.
Main duties of the job
This is a challenging role requiring the post holder to work with patients, families, clinical and non-clinical staff groups including colleagues from Local Authority, Continuing health Care/Personalised Commissioning, Bradford District Care Trust and the various Multi-Agency Support teams.
The post-holder will work across BRI, SLH and the community hospital sites.
The post covers 7 days a week; weekend working will be on a rota basis
You should be a Registered Nurse or Allied Health professional at least relevant registration experience that includes involvement with complex discharge planning for patients.
