This fixed-term post is an exciting opportunity to join BTHFT's well-established & award winning multidisciplinary Elderly Virtual Ward Team - delivering consultant led care closer to home.
Main duties of the job
The post holder will contribute to seamless care across organisational boundaries. This means reviewing patients from an acute setting to a community setting in order to improve the quality of patient care; to promote evidence-based and cost-effective prescribing. Blurred boundary working is a key part of the team's success - and there is an opportunity to develop additional skills not traditionally associated with pharmacy.
The post holder will utilise their independent prescribing qualification to direct the MDT in medication review/ decision making and provide patients with support in their own home. There is a particular focus on medicines optimisation, rationalisation and adherence.
It is essential that you have a full UK driving licence and have a car available for work
